Appointments are given for both initial and follow-up visits. The appointment indicates the time patients are expected to arrive for eligibility screening and triage, not the time they will be seen by the physician. Walk – Ins (patients without an appointment) are allowed to sign in on a stand-by list and are seen on a “space available basis” only. Walk-in patients may be seen in the event of cancellations or “no shows.” Patients requiring urgent care are referred to the ER.
The Community Health Clinic was established to provide access to health care for those residents of Hardin and LaRue Counties who are employed, yet unable to afford health care and do not qualify for government medical programs, e.g. Medicare, Medicaid.

All patients seen at the clinic must be residents of Hardin or LaRue County , without health insurance, and qualify for services under one of the following criteria:
•  Persons gainfully employed, who meet the income criteria, and their dependents
•  Persons who are full-time students
•  Persons receiving disability income without insurance and their spouses
•  Persons unemployed less than 12 months
•  Persons claiming child support as sole source of income
•  Persons moving to Kentucky awaiting Medicaid Benefits

NOTE: Persons living in the household who are not related and are not dependents will not be considered to qualify or disqualify others in the home.

EXCEPTIONS: Exceptions are made by the Medical Director, e.g. unemployed persons with certain medical conditions.

All patients must complete the registration form and provide:
•  Photo Identification
•  Proof of residency
•  Proof of household income
The Community Health Clinic of Hardin - Larue Co. requires household income be at or below 185% of the 2005 Federal Poverty Guideline.
