All services are provided free of charge and are available only to those persons meeting clinic eligibility requirements and begin with a medical office visit by which additional services may by provided with a medical order.
A volunteer staff of physicians, physician assistants, and nurse practitioners provide medical office visits every Tuesday and Thursday evening. Patients must be willing to be seen by whatever medical staff on duty.
The Community Health Clinic hosts its own licensed pharmacy providing pharmacy services ordered during a medical office visit. Prescriptions written outside the clinic are not dispensed by the clinic pharmacy with the following exceptions, e.g. clinic patients issued prescriptions through a hospital Emergency Room visit and those issued by a specialist consultation arranged by the clinic.
The clinic lab is CLIA waived and sponsored by Lab Corp. Volunteer lab technicians collect specimens on Tuesdays and Thursdays between 6 p.m. and 8 p.m.
Clinic patients must be evaluated during a medical office visit first before being referred for dental care. The Community Health Clinic volunteer dentists provide basic dental services at the U of L Dental office located at 1010 Woodland Drive, Suite 204. Due to the high demand for dental services, dental referrals must be placed on a waiting list. Urgent referrals are given priority.
Through an affiliation with the College Lions Club clinic patients may receive a referral for Vision Care. The College Lions Club provides vouchers for local optometry services and basic eyewear.
Certain diagnostic services ordered through a clinic office visit may be made available free of charge or at a substantial discount. However, patients are financially responsible for any testing procedures scheduled at Hardin Memorial Hospital. The HMH Financial Services Department offers financial assistance programs to those who qualify.
Local specialists provide evaluation and recommendations for clinic patients referred for a one visit courtesy consultation. Specialist consultations unavailable locally are referred to U of L Physicians Associates where services are provided on a sliding scale.
Home medical equipment is provided free of charge to clinic patients with medical certain conditions, e.g. asthma, sleep apnea.
